At New Found Hope, we offer a range of services to help individuals live independently and confidently. Our 24-hour emergency assistance ensures help is always available, while adult companion services provide emotional support and companionship. We maintain safe, organized homes with homemaker services, and teach individual community living skills to help individuals engage confidently in their communities. Our personal support services assist with daily tasks, and semi-independent living space skills strike the perfect balance between independence and support, enabling individuals to thrive.

Eligibility

Individuals receiving services must be enrolled in one of the following waiver programs.​

Alternative Care (AC)

Brain Injury (BI)

Community Alternative Care

Community Alternatives for Disabled Individuals (CADI)

Developmental Disability (DD)

Elderly Waiver (EW)

Referral Process

At New Found Hope, the referral process is designed to ensure that individuals receive the appropriate care and support tailored to their needs. To access our services, individuals must be referred by a case manager and meet specific eligibility requirements, such as having the necessary waiver for programs like Brain Injury (BI), Community Alternative Care (CAC), Community Access for Disability Inclusion (CADI), or Developmental Disabilities (DD). This process ensures that every person receives the right level of care and resources to enhance their independence and overall well-being.
