All 245D Basic Services require that an individual holds a waiver for one of the following programs: Brain Injury (BI), Community Alternative Care (CAC), Community Access for Disability Inclusion (CADI), or Developmental Disabilities (DD). Additionally, individuals must be referred by a case manager to ensure they receive the appropriate care and support. These requirements are in place to ensure that individuals receive the services tailored to their specific needs and are supported in achieving their highest level of independence and well-being.
